Perfectionism is anxiety in a nicer outfit: a therapist's guide for high-achieving women

Perfectionism isn't a personality trait — it's often anxiety with a résumé. Here's how to recognize it, why it's exhausting, and how therapy for women helps you loosen the grip.

Soft botanical still life — a calm image for a post on perfectionism and anxiety in women

Perfectionism has a good publicist. It gets called 'high standards,' 'attention to detail,' 'just how I am.' It shows up on résumés and in performance reviews. It gets you promoted. What it also does — quietly, expensively — is keep your nervous system in a low-grade emergency for years at a time.

For a lot of the working women I see across New York and New Jersey, perfectionism is not a personality trait. It is anxiety wearing a nicer outfit.

What perfectionism actually is

Clinically, perfectionism is the belief that your worth is contingent on flawless performance, and that any mistake will cost you love, safety, or belonging. That belief isn't a quirk. It's a survival strategy that a younger version of you learned in an environment where being 'good' was the safest way to be.

How to tell perfectionism from healthy standards

  • Healthy standards feel like pride when you meet them. Perfectionism feels like relief — briefly — then the bar moves.
  • Healthy standards let you rest. Perfectionism treats rest as evidence you're falling behind.
  • Healthy standards let you take feedback. Perfectionism experiences feedback as a threat to your identity.
  • Healthy standards allow for a range of outcomes. Perfectionism has only two: exceptional or unacceptable.

Why perfectionism is so exhausting

Because you are not just doing the task. You are doing the task, monitoring how you're doing the task, pre-empting every critique of the task, and rehearsing what to say if the task falls short. That is four jobs. Of course you're tired.

"You are not tired because you're weak. You are tired because you have been doing four jobs to accomplish one."

What actually helps (and what doesn't)

'Just lower your standards' is not therapy. Real work on perfectionism happens on three layers: the thoughts, the body, and the story underneath. Cognitive reframing helps interrupt the loop. Somatic and nervous-system work teaches your body that a B+ is not, in fact, a threat. And deeper story work asks: who taught you that love had to be earned this hard?

  1. 1.Track the moment the bar moves. Notice the exact second 'good enough' becomes 'not enough.' That is the pattern, live.
  2. 2.Practice deliberate imperfection in low-stakes places. Send the email with one typo. Leave the dishes overnight. Notice the world does not end.
  3. 3.Separate output from identity. You are not your last performance review.
  4. 4.Get support that treats perfectionism as anxiety, not personality. This is what individualized anxiety therapy is for.
